FORMS

• Clinical Experiences Student Placement Form  – to be submitted initially by ALL traditional teacher candidates before the end of the second week of their Field Experience 1 course. Students should then resubmit any time there is a change to the information, especially changes in mailing address, contact info, program, endorsements, or means of transportation.
• Request for Clinical Partnership – to be submitted by teacher candidates who are currently employed as full-time substitutes, instructional aids, or in other classroom or institutional support roles at grade or age levels appropriate for their anticipated Professional Educator License (PEL), in order to complete required field experiences at their location of employment, subject to the approval.
• Waiver of Clinical Hours – to be submitted by educators currently teaching full-time in the area for which they are seeking endorsement (bilingual and/or ESL) and have been doing so for at least three months.
• Field Experience Time Logs-Tk20 Login Page – for tracking field experience hours for teacher candidates in Field Experience 1, 2 and 3 courses, and Instructional Coaching 1 & 2 courses.
